Baptist Health Home Care, a Medicare-certified and Joint Commission-accredited home health agency, continues to surpass state and national averages on several key quality measurements, as reported by the Centers for Medicare/Medicaid Services (CMS). CMS recently released its most current report of clinical outcome measurement scores, and Baptist Health Home Care reaffirmed its commitment to providing quality care.

“Our clinical staff’s vast experience allows them to better look for and prevent complications that lead to re-hospitalization,” Maria Zamora, director of Baptist Health Home Care, said in a news release. “Our goal is to empower our patients in the disease management process and to prevent hospital readmission whenever possible.”

CMS reports that the 85% of Baptist Health Home Care’s patients stayed at home — avoiding hospital readmission — after an episode of home health, compared to only 68% nationwide. Only 14% of Baptist Health Home Care’s patients had to be readmitted to the hospital, 15 percentage points better than the national average of 29%. In addition, the national average percentage of patients who needed urgent, unplanned medical care was 22%, compared to Baptist Health Home Care’s lower rate of 14%.
